This book investigates the impact of patient-centered care (PCC) for older adults with chronic conditions and presents new evidence concerning its effects on the care process and health outcomes. The book highlights the impact of PCC on elderly chronic disease patients' health experiences and demonstrates that PCC is associated with better performance in delivering care to this group of patients. The PCC approach also provides opportunities for improving the delivery of cancer care, although patient-centered oncology care is still in its infancy and evidence on its effectiveness is scant. Hailun Liang, Ph.D., is an Assistant Professor in health policy and management at the School of Administration and Policy of Renmin University of China. She is also an Adjunct Assistant Professor at the Johns Hopkins Bloomberg School of Public Health. In 2016, she obtained her doctoral degree in health policy and management from the Johns Hopkins Bloomberg School of Public Health. Her research focuses are primary care, health equity, and chronic disease management. She has published articles in scholarly venues such as the Medical Care, Health Services Research, and International Journal for Equity in Health.
