Atherosclerosis and spondyloarthritis: what's the link?

This book explores the complex links between atherosclerosis and spondyloarthritis, two chronic pathologies whose interaction significantly worsens patients' cardiovascular prognosis. It highlights the central role of systemic inflammation in the accelerated development of atherosclerosis in patients with spondyloarthritis, particularly axial spondyloarthritis. Through an in-depth analysis of immuno-inflammatory mechanisms, such as the activation of pro-inflammatory cytokines (TNF- , IL-17), the book demonstrates how these mediators contribute to endothelial dysfunction, increased oxidative stress, and the formation of unstable atheromatous plaques.The book draws on clinical, epidemiological and vascular imaging (carotid, coronary) data to illustrate the high frequency of subclinical atherosclerosis in this population. It also discusses the limitations of conventional cardiovascular risk scores, which are often ill-suited to these young, inflammatory patients.Finally, the book stresses the importance of integrated management, combining control of inflammatory activity (biotherapies, NSAIDs) and rigorous management of cardiovascular risk factors (smoking, dyslipidemia, sedentary lifestyle). It offers practical recommendations for screening and prevention, paving the way for personalized medicine aimed at reducing cardiovascular mortality in spondyloarthritis.
