Comprehensive Diabetes Care For Elderly

Home Health Care Services (HHCS)provide elderly with all rounded medical and health care services in the discretion and luxury of their home. The HHCS has a mission that aims at promoting and maintaining the standards of elderly care medicine and care for complex cases as the foremost organization in the country itself. Diabetes Mellitus is one of the major problems needs to be addressed in HHCS settings currently. Out of 1200 patients under HHCS 600 is suffering from Diabetes. As the project lead,the author intends to take up and delegate the responsibility of providing coaching and educating the members in the team about the Local Diabetes guidelines with regards to monitoring and Management of Type 1 and Type 2 Diabetic Mellitus. Subsequently,the team will educate all the HHCS staff including nurses, physicians,and other healthcare providers and the timely completion of various phases of the project in the proposed time frame. The project followed by a detailed discussion on the implementation process itself,discussed its effect on the organization and the conclusions deduced following execution of the Diabetes Project.
