Over centuries now Southern Regions of Tanzania suffer with poor performance of form four examination. The research was done at Mtwara-Mikindani Municipal and come out with the factors contribute to poor performance in secondary schools were shortage of learning and teaching materials, undesirable cultural practices, moral deterioration and truancy. Where by a researcher recommended that Government, Teachers, Parents and other stakeholders should be responsible to the performance of the secondary students through highly commitment by performing their duet and responsibility towards education such as government should allocate teacher, teaching and learning material to the school, teachers should maintain discipline of students, parents should attending the school meeting to know the progress of their children, and the stake holders should provide teaching and learning material to the school which have insufficient teaching an learning material as a result the problem will be solved.
