Meghalaya's MOTHER project (2019) was evaluated using a convergent mixed-methods design to assess changes in six maternal and child health indicators-MMR, IMR, institutional deliveries, full immunization, ANC ( 4 visits), and first-trimester registration-comparing a pre-2019 baseline (2015-18/2016-18) with 2021 and 2024. Quantitative trends were drawn from NFHS-4, SRS and the state "MegHealth" dashboard, with descriptive analyses of absolute and relative changes. Qualitative insights came from ten purposively sampled in-depth interviews with frontline health workers (March 2024); thematic analysis of field notes highlighted community practices, system-level strengthening, and persistent barriers affecting service uptake.
