Industrial accidents represent an unfavorable aspect and are threat to human life, equipment and environment. Petroleum refineries are extremely dangerous Wood & Gene's, et al. (2015), the consequences of any event can be disastrous at both human and environmental levels, as well as cost millions of dollars. The hazards are due to the presence of hundreds of tons of oil inside the various product lines; which are generally flammable and poisonous to humans and to the surroundings; and under such conditions, an out of control coincidence may have giant results in the immediate vicinity Christou etal. (1999). The outcomes of such events might affect not only the industrial sites but also have environmental and socioeconomic influences (Kadri and Châtelet, 2013). Every year, industrial accidents cause a number of deaths, injuries, and asset losses due to petroleum refining operations. The technique of learning from injury occurrences is often mentioned as one of the pillars of modern approaches to risk control measures Dechy etal. (2008) that aid in preventing the fate from recurring.
