Safety is a none event! Every oil and gas activity is safe until an unsafe human error triggers a precipitating event that culminates in disaster. The last unsafe act is never the actual cause of a disaster event. The disaster of Deepwater Horizon could have been avoided. The impact of organisational safety culture on disaster prevention can never be over-emphasized. An evolving and flexible safety culture that rises to meet the increasing complexity in deepwater exploration is pertinent in disaster prevention.
